Job Summary:
A Care Manager (CM) is a trained healthcare professional who works both within the administrative and medical capacities to ensure the smooth running of the establishment that they are working with. The typical responsibilities of the CM include effective management and implementation of the population health management services that are being provided at the establishment; development of and building up the capacity of the team members; overseeing the day-to-day management; ensuring appropriate documentation, reporting, and associated billing.
Responsibilities include but are not limited to:
Imbibing, developing, and implementing the complete aspects of Chronic Care Management (CCM) and Remote Patient Monitoring (RPM) services with the care team (Practice manager, Primary care physician, nursing staff, medical assistant, Clinical Coordinators CC, and Clinical Facilitators (CF).
- To manage the assigned clinics efficiently through exemplary leadership, guidance, and control.
- To ensure that quality assurance mechanisms are in place and are being adhered to.
- To ensure practical implementation of QA action plans and take corrective actions to improve services.
- To allocate and manage the workload of CCs with an assignment of daily tasks.
- To manage & resolve grievances within the team/department and play a facilitative role in inter-department coordination.
- To provide extensive feedback and support to the CC in carrying out all services applicable to the clinic.
- Effectively coordinate and exhibit teamwork while working across different teams of Care Coordinators, Care Facilitators, and PEM team members in overcoming operational challenges.
- To conduct monthly progress review meetings with the Clinics team and develop a business relationship exhibiting 2C health solutions values.
- To ensures timely submission of accurate Monthly Progress Reports & End of Day (EOD) sheet.
- Conduct and manage relationships at their respective clinic through regular communication, presentation, and performance evaluation after approvals.
- To oversee, ensure and help the team that appropriate documentation is collated, summarized, entered, and communicated with the care team in all relevant/important sections of EMR and the 2C portal for all services offered at their respective clinic.
- Ensuring that appropriate services /financing-related data is being maintained, generated, and invoiced on time.
- Ensures compliance with Insurance standards for the provision of services to clients in the clinics.
- Pursue quality services at the clinic with a patient focus approach and implement tools for patient and clinic feedback monitoring.
- Identify & address areas of improvement for team members with regular communication with Team Leads and bridge gaps through job capacity-building sessions via respective departments.
- Ensuring availability for CC meetings, Clinic Liaison meetings in the evening, and answering CC queries on Signal till a reasonable time at night (it's expected given the flexibility of off hours during the day for the team).

Qualification:
MBBS from a recognized institution along with valid PMC Registration.
Experience:
Minimum 1 – 2 years of experience working in clinical settings with active interaction with patients and exposure to Health Care Management.
Timings in PST :
Morning: 09:30 a.m - 05:00 p.m / Night: 08:00 p.m - 04:00 a.m
Location: Work From Home
